Please complete this Registration Form BEFORE going to the office with your documents.

You must complete the process at the office; we are not aware that you have registered, and your student is not enrolled, until you complete Step 3.

We will make copies of your documents and/or print out any forms or documents for you - you do not need a printer. 

Elementary (Kindergarten - 5th grade)

• Original Birth Certificate (Birth Certificate: Order Online)

• Current Immunization Record or Health Department Waiver - this can be faxed directly to us from your provider's office

• Proof of Residency - see Residency Verification for required/acceptable documents

• Y5/Kindergarten: proof of a hearing & vision screening** - this can be faxed directly to us from your provider's office

• Y5/Kindergarten: proof of a dental exam - this can be faxed directly to us from your provider's office

• Documents requiring your signature will be provided to you at the building

• If your Y5/Kindergarten student hasn't been given an assessment at their elementary building, please bring them with you to enrollment!

Middle School (6th - 8th grade):

• Original Birth Certificate (Birth Certificate: Order Online)

• Current Immunization Record or Health Department Waiver - this can be faxed directly from your provider's office

• Proof of Residency - see Residency Verification for required/acceptable documents

• Recent Report Card

• Mona Shores Middle School Elective Sheet (this form will be provided for you at the office)

• Documents requiring your signature will be provided to you at the building

• 2023-24 MSMS Course Guide

High School (9th - 12th grade):

• Original Birth Certificate (Birth Certificate: Order Online)

• Current Immunization Record or Health Department Waiver - this can be faxed directly from your provider's office

• Proof of Residency - see Residency Verification for required/acceptable documents

• Recent Transcript (or 8th grade report card for 9th graders)

• Course Request Form (click on link below for forms by year of graduation)

• Documents requiring your signature will be provided to you at the building
