Please complete the Registration Form the day before you plan to visit the office with your documents. You must complete the process at the office; we are not aware that you have registered, and your student is not enrolled, until you complete Step 3.

We will make copies of your documents and/or print out any forms or documents for you - you do not need a printer. 

Elementary (Kindergarten - 5th grade)

• Original Birth Certificate (Birth Certificate: Order Online)

• Current Immunization Record or Health Department Waiver - this can be faxed directly from your provider's office to the building

• Proof of Residency - see Residency Verification for required/acceptable documents

• Y5/Kindergarten only: proof of a hearing & vision screening

• Y5/Kindergarten only: proof of a dental exam

• Documents requiring your signature will be provided to you at the building

Middle School (6th - 8th grade):

• Original Birth Certificate (Birth Certificate: Order Online)

• Current Immunization Record or Health Department Waiver - this can be faxed directly from your provider's office to the building

• Proof of Residency - see Residency Verification for required/acceptable documents

• Recent Report Card

• Mona Shores Middle School Elective Sheet (this form will be provided for you at the office)

• Documents requiring your signature will be provided to you at the building

High School (9th - 12th grade):

• Recent Transcript (or 8th grade report card for 9th graders)

Please go to High School Class Scheduling Information for graduation requirements and course information.  Course request forms are available on this page if you would like to print it out and bring it to your new student appointment with a parent/guardian and your counselor, or one will be provided to you at the appointment.

Summer enrollment for the 2023-24 school year will begin at the Administration Office on Monday, July 17, 2023.

The first day of school is Tuesday, September 5, 2023. You must complete enrollment by August 30, 2023, in order to start on the first day!

If you do not complete enrollment by 4:00 pm on August 30, 2023, your child will not be able to start school on the first day.
