Student Enrollment Process/Forms

Welcome to Mona Shores Public Schools!

We look forward to working with you and your child as you progress through these very important educational years!  Please see below for important information regarding the enrollment process and required documentation.  Please be aware that your child may not attend class until the proper documentation is on file with the school district.

It is always our goal to provide your child with an excellent education in a safe environment.  You are our partner in this important endeavor and we welcome you to the Mona Shores family!

Enrollment Process:

Step 1 - Online Registration

NOTE:  The online enrollment form requires an overnight to process to our database.  Please complete the form one business day before you intend to visit your building office with your documents.

 

Step 2 - Gather Required Documents

We will make copies of your documents and/or print out any forms you need at the building.

Elementary (Kindergarten - 5th grade)

Middle School (6th - 8th grade):

For new 6th, 7th, & 8th grade students, please print and complete the appropriate grade-level elective sheet & the Getting to Know You page:

High School (9th - 12th grade):

  • Original Birth Certificate
  • Current Immunization Record (Request for Michigan Immunization Record)
  • Proof of Residency - See the Residency Verification Form for required/acceptable documents
  • Recent Transcript (or recent 8th grade report card)
  • Course Request Form (click on link below for forms by year of graduation)
  • Other documents requiring your signature will be provided to you at the building

Step 3 - Take your required documents to the building

Elementary:  main office at the building for the attendance area in which you live

Middle school:  counseling office at Mona Shores Middle School, 1700 Woodside Rd

High school:  counseling office at Mona Shores High School, 1121 Seminole Rd
