Enrollment for health, dental, vision and life benefits is the first of the month following a 90 day probation period. To commence the 1st of the month following completion of probation.

 

Bus Driver personnel may enroll single subscriber for health beneifts and for dental and vision their spouse and dependents are elgible on based on the need of family status.

 

Health - MESSA Super Care I

Claims filed by calendar year

Deductible is 100/200 – 90% paid after deductible is met

Prescription co-pay - $5.00 generic/$10.00 name brand

OR

 

Health - MESSA CHOICE II



 

 

Employees not electing health insurance will receive one humdred Dollars ($100.00) per month in accordance with the district's Section 125 of the IRS Code 1986.

Dental - Delta Dental Plan of Michigan

The employer shall provide for each employee, his/her spouse, and his/her dependents the current dental plan.

 

Class I Benefits   -  Basic Dental 60%

Class II Benefits  -  Prosthodontic Services 60%

Class III BenefIts  - Orthodontic Services (to age 19) 50%

 

Maximum payment: $1000 per person total per benefit year on Class I and Class II benefits. Delta Dental's payment for Class III benefits will not exceed a lifetime maximum of $500 per eligible person.

 

  

Vision Insurance – Vision Service Plan / VSP2

Benefit year is April 1 through March 31

Can use panel provider or non-panel provider doctors. You receive a better benefit when using a panel provider.Call Insurance office for non-panel provider claim forms.
