MESSA ABC Plan 2 Coverage Guide

  • Benefit year is January 1 - December 31.
  • Annual deductible is $2,000 for Single coverage and $4,000 for 2-Person and Full Family coverage.
  • MESSA ABC Rx Prescription Drug Coverage
  • After the applicable deductible is met, copayments are:
    • $2 for generic maintenance medications for specific chronic conditions and diseases.
    • $10 for all other generic medications.
    • $20 for specific brand name maintenance drugs used to treat diabetes and asthma.
    • $40 for brand name drugs when no generic product exists.
    • Please refer to your Plan Coverage Booklet for full details, limits and exclusions.
  • Health plan includes a $5,000 Basic Term Life insurance policy. 

Dental Benefits

Set Seg Dental Plan

  • Benefit year is January 1 - December 31.
  • Includes two cleanings per year.
  • Annual maximum is $1,000.
  • Orthodontic lifetime maximum is $1,500.

Vision Benefits

Set Seg Vision Plan

  • Benefit year is July 1 - June 30.
  • Examinations, frames and one set of corrective lenses will be provided once in a 12-month period, defined as July 1 to June 30, for each eligible member.
  • Vision Plans provide benefits for eligible expenses at 100% of reasonable & customary fees or at follows:
    • Vision Exam $55.00
    • Single-vision lenses $73.00
    • Bifocal-vision lenses $84.00
    • Trifocal-vision lenses $100.00
    • Lenticular-vision lenses $124.00
    • Contacts prescribed after cataract surgery or when vision is not correctable to 20/70 in the better eye except by their use. $175.00
    • Contact lenses in lieu of glasses not to exceed: $110.00

Life Insurance Benefit

Provided through Dearborn National.

  • $150,000 Life and $150,000 AD&D benefit.

Long Term Disability Benefit

Provided through Dearborn National.

  • Benefit begins after 90 days of employment.
  • 66 2/3% of contract  - Maximum eligible monthly salary $5,000.

Optional Benefits

Short Term Disability, Long Term Disability, Supplemental Term Life Insurance, Dependent Life Insurance and Survivor Income Insurance is available through MESSA and Set Seg.  Enrollment is available during the first 30 days of employment or annually during the Open Enrollment process.  These additional benefits are paid for by the employee.
